C-HOBIC is a joint project between the Canadian Nurses Association and Canada Health Infoway to begin the process of collecting standardized evidence based clinical outcomes for inclusion in electronic health records. St. Boniface Hospital, as the regional pilot site, has embedded standardized questions about function, symptoms, safety outcomes and readiness for discharge into admission and discharge assessments. This information can assist clinicians in evaluating the impact of their practice on clinical outcomes. For example, clinicians can view standardized information from the admission and discharge assessments to see if a patient's health status has improved in the areas of function, symptoms, etc.
The C-HOBIC Transitions Summary Report is produced on discharge to provide clinical information to health care providers to support continuity of care across the continuum.
