A new approach to placements in personal care homes

Client centred approach will focus on meeting individual needs

Waiting times between panel and placement in a person care home can range from one week to 10. And if the placement isn't a good fit, it's disruptive for the person and the personal care home.

The solution? Long term care panel re-design. The new approach is client centred, and focused on what a person needs and what's most important in their placement in a personal care home.
"This particular project has guiding principles that goes across sectors," says Gina Trinidad, Chief Operating Officer of Long Term Care and Deer Lodge Centre. "It means having timely admissions based on a person's needs."

Instead of being placed based on panel date, a person will be placed based on the timeframe of their need for placement. That wait may be as short as 48 hours but will not exceed six months. Longer waits will likely indicate a greater probability a person is placed in their preferred personal care home.

"We don't really know the impact of a wait on people. Even an extra day in the hospital could result in someone with compromised immunity picking up a bug. When we work in our own silos, we don't think about those things," says Trinidad.
