Health Outreach and Community Support (HOCS)

FEATURES

Population:

  • Individuals who are experiencing homelessness or are unstably housed who:
    • have under/un-resourced concerns about health and safety, and/or
    • require a clinically complex response to support plans for stability.

Program Features:

  • Clinical consultation, with a focus on access to health & social services for individuals who are:
    • Consultation support to enhance existing support services’ ability to improve housing and health stability
    • Direct service, including assessments, to assist with care plan development or connection to longer term supports
    • Collaboration in care planning
    • Capacity building for service provision in the homelessness sector

Team Involved:

  • Clinical Coordinators (Social Work, Occupational Therapy, Nursing)
  • Trauma Worker
  • Community Services Coordinator (Outreach)
  • Psychiatry
  • Psychology

REFERRAL CRITERIA

  • Individuals who are homeless or unstably housed and:
    • their existing care team resources have been contacted and require more support;
    • have experienced challenges receiving coordinated health or social services;
    • frequently access emergency services, have long term experiences of homelessness, or high risk
