Health Outreach and Community Support (HOCS)
FEATURES
Population:
- Individuals who are experiencing homelessness or are unstably housed who:
- have under/un-resourced concerns about health and safety, and/or
- require a clinically complex response to support plans for stability.
Program Features:
- Clinical consultation, with a focus on access to health & social services for individuals who are:
- Consultation support to enhance existing support services’ ability to improve housing and health stability
- Direct service, including assessments, to assist with care plan development or connection to longer term supports
- Collaboration in care planning
- Capacity building for service provision in the homelessness sector
Team Involved:
- Clinical Coordinators (Social Work, Occupational Therapy, Nursing)
- Trauma Worker
- Community Services Coordinator (Outreach)
- Psychiatry
- Psychology
REFERRAL CRITERIA
- Individuals who are homeless or unstably housed and:
- their existing care team resources have been contacted and require more support;
- have experienced challenges receiving coordinated health or social services;
- frequently access emergency services, have long term experiences of homelessness, or high risk
