Pulmonary Rehab

Program Features:

• eight-week program delivered in a group setting
• offers individualized exercise programs to improve breathing and tolerance
• includes education to understand lung disease, prevent infections, learn breathing and relaxation techniques, and use medications effectively
• the goal of the program is to assist clients with self- management of their lung problems
• the program also offers “stop smoking strategies”

Team Involved:

• Variable by site but may include OT, PT, SW, RT, RA, Dietitians, Pharmacists, Kinesiologists, Respirologists

• clients with chronic obstructive pulmonary disease (COPD), emphysema, chronic bronchitis, pulmonary fibrosis, and long COVID

Referrals are accepted from Primary Care Providers, Home Care Case Coordinators, Hospital Inpatient Units and Emergency Departments
