Step 4: Evaluate
Evaluate and revise the Action Plan
It is suggested that the team meet every three to six months to review and evaluate progress of the action plan. During these meetings, it is important to revisit the plan for any changes or revisions needed and to celebrate successes and accomplishments. The journey towards excellence in Collaborative Care is an ongoing process that will require adjustments and changes to the action plan throughout time.
